Banty Dental Corporation, d/b/a Sola Kids Dental & Orthodontics, 4021 S Central Ave, Los Angeles, CA 90011.

2. Eligibility

Open to legal residents of the State of California who are eighteen (18) years of age or older at the time of entry. Employees, contractors, and owners of Banty Dental Corporation, its d/b/a Sola Kids Dental & Orthodontics, and its affiliated dental practices, along with their respective immediate family members (parents, spouses, siblings, children, and step-relations of the same) and members of the same household of any such person (whether related or not), are not eligible to enter or win.

3. Entry Period

Sweepstakes begins July 6, 2026 at 12:00 AM Pacific Time and ends July 20, 2026 at 5:30 PM Pacific Time (the “Entry Period”). Entries received before or after the Entry Period will not be accepted.

4. How to Enter

You may enter using any one of the following methods:

(a) Online Entry: Complete the official entry form at solakidsdental.com/grand-opening/west-adams/sweepstakes.

(b) In-Person Entry: Complete an entry card at any Sola Kids Dental & Orthodontics office during regular business hours.

Limit one (1) entry per person per method. Multiple entries beyond this limit will be disqualified. Automated entries, entries generated by script, macro, or other automated means, and entries obtained through fraudulent methods will be disqualified. Illegible, incomplete, or damaged entries will be disqualified.

5. Prizes

Grand Prize (1 winner): Complete Orthodontic Treatment.

One full course of comprehensive orthodontic treatment at any Sola Kids Dental & Orthodontics office in Los Angeles County. Prize includes: initial consultation, comprehensive clinical evaluation, treatment planning, orthodontic appliances (traditional braces), all scheduled follow-up visits and adjustments through completion of active treatment, and one set of retainers upon completion.

Treatment must be performed at a Sola Kids Dental & Orthodontics office and completed within thirty-six (36) months of prize award date. Approximate retail value (ARV): $5,500, excluding pre-treatment or post-treatment restorative work not included in the standard treatment plan.

Clinical Candidacy Requirement. The Grand Prize is subject to a clinical candidacy determination made by a California-licensed orthodontist practicing at Sola Kids Dental & Orthodontics. The candidacy determination is made in the sole and reasonable clinical judgment of the treating orthodontist and considers factors including but not limited to: dental and periodontal health, bone structure, patient age and treatment feasibility, and any conditions that would contraindicate the specific treatment described. The candidacy determination is final.

If the winner is determined not to be a clinical candidate for the orthodontic treatment described, or if the winner declines to proceed with a clinical evaluation within thirty (30) days of prize notification, the winner will receive, in lieu of the orthodontic prize, a five hundred dollar ($500) credit toward alternative dental services at any Sola Kids Dental & Orthodontics office. This credit is non-transferable, cannot be redeemed for cash, and expires twelve (12) months from the date of award.

Second Prize (1 winner): Apple iPad.

Apple iPad, model and configuration selected by Sponsor at time of award. Latest generation available. Approximate retail value (ARV): $499.

​

Prize Restrictions. Prizes are non-transferable, non-refundable, and cannot be exchanged for cash. No substitutions except by Sponsor, who reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable.

​

Grand Prize Exclusions.

The Grand Prize does not include, and Sponsor is not responsible for providing:

(a) any pre-treatment dental services required before orthodontic treatment can commence, including but not limited to cleanings, restorative work, extractions, periodontal treatment, or oral surgery;

(b) any post-treatment restorative or cosmetic work following completion of orthodontic treatment;

(c) treatment of orthodontic conditions that arise outside the initial approved treatment plan or that develop after treatment commencement;

(d) replacement of lost, broken, or damaged appliances or retainers, except as required by manufacturer warranty during the active treatment period;

(e) treatment extending beyond thirty-six (36) months from the prize award date; and

(f) any related medical or dental services provided by any provider other than Sola Kids Dental & Orthodontics.

Winner is solely responsible for all federal, state, and local tax obligations arising from receipt of the prize. Sponsor will report prize value to tax authorities as required by law. Winner may be required to complete IRS Form W-9 as a condition of receiving the prize.

6. Winner Selection

Winners will be selected by random drawing from all eligible entries on July 20, 2026, following the close of the Entry Period, at the Sola Kids Dental & Orthodontics grand opening event located at 5189 W Adams Blvd, Los Angeles, CA 90016. Odds of winning depend on the total number of eligible entries received.

7. Winner Notification

Winners are not required to be present at the grand opening event to win. Winners will be announced at the grand opening event on July 20, 2026, and will also be notified by phone and email within five (5) business days of the drawing. If a winner cannot be reached within ten (10) business days of the first notification attempt, or if a winner does not respond to notification within ten (10) business days, an alternate winner will be selected from remaining eligible entries.

8. Winner Requirements

As a condition of receiving any prize, each winner must, within thirty (30) days of first notification attempt:

(a) confirm eligibility under Section 2 of these Official Rules;

(b) sign and return an Affidavit of Eligibility and Liability Release in the form provided by Sponsor;

(c) where permitted by law, sign and return a Publicity Release in the form provided by Sponsor;

(d) provide a completed IRS Form W-9 if the prize value exceeds federal reporting thresholds;

(e) provide a valid government-issued photo ID confirming California residency and age; and

(f) for the Grand Prize only, complete a clinical evaluation at a Sola Kids Dental & Orthodontics office in Los Angeles County.

Failure to complete any of the above requirements within the thirty (30) day period, or return of any notification as undeliverable, will result in forfeiture of the prize. In the event of forfeiture, Sponsor may, at its sole discretion, select an alternate winner from remaining eligible entries or elect not to award the prize.

9. Publicity and Right of Publicity

Except where prohibited by law, acceptance of a prize constitutes each winner’s consent for Sponsor and its affiliates, licensees, and assigns to use the winner’s first name, last initial, city of residence, photograph, likeness, statements made about the prize or Sponsor, and prize won for advertising, promotional, and publicity purposes in any media now known or hereafter devised, worldwide, in perpetuity, without further notice, review, approval, or compensation.

California residents have certain rights under California Civil Code Section 3344 regarding use of their name, voice, signature, photograph, or likeness for commercial purposes. By accepting a prize, the winner grants Sponsor the specific consent required under Section 3344 for the uses described above and waives any claim for additional compensation.

Any use of a winner’s full name, precise address, or likeness in a manner not described above requires separate written consent from the winner.

10. General Conditions

By entering, participants agree to be bound by these Official Rules and by the decisions of the Sponsor, which are final and binding in all matters relating to this Sweepstakes. Sponsor reserves the right to disqualify any entrant who tampers with the entry process, violates these Official Rules, or acts in an unsportsmanlike or disruptive manner. Sponsor reserves the right, in its sole discretion, to modify, suspend, or terminate the Sweepstakes in the event of technical failures, fraud, or any other cause that materially affects the administration, security, fairness, or integrity of the Sweepstakes.

11. Release

By entering, participants release and hold harmless Sponsor, its parent companies, subsidiaries, affiliates, and their respective officers, directors, employees, and agents from any and all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property that may be sustained in connection with the receipt, ownership, or use of any prize, or participation in this Sweepstakes.

12. Governing Law

This Sweepstakes is governed by the laws of the State of California, without regard to conflict of law principles. Any dispute arising from this Sweepstakes shall be resolved exclusively in the state or federal courts located in Los Angeles County, California, and participants consent to the jurisdiction of such courts.

13. Void Where Prohibited

This Sweepstakes is void where prohibited or restricted by law.

14. Winner List

15. Privacy and Data Use

Personal information collected from entrants in connection with this Sweepstakes will be used by Sponsor to administer the Sweepstakes, contact winners, comply with tax and legal reporting obligations, and, only where the entrant has separately opted in, to send marketing communications from Sola Kids Dental & Orthodontics.

Sponsor’s collection, use, and disclosure of personal information is governed by Sola Kids Dental’s Terms of Use, available at solakidsdental.com/termsandconditions, and by the California Consumer Privacy Act as amended by the California Privacy Rights Act (collectively, the “CCPA”).

California residents have specific rights under the CCPA regarding personal information collected about them, including the right to know what personal information is collected, the right to request deletion, the right to correct inaccurate information, the right to opt out of the sale or sharing of personal information, and the right to limit the use of sensitive personal information.
